Business Name Reconsideration

Business Name Reconsideration is a process designed to retain your business name if, after failing to renew within the 5-year certification period and the subsequent 6-month grace period. In the event that, during an attempt to register your business name as new, it fails in validation, and you possess the necessary requirements, you have the opportunity to undergo Business Name Reconsideration.

  • Letter of Request for business name, addressed to OIC PD Lilibeth R. Chavez
  • Accomplished Application Form signed by the owner
  • Supporting Documents (copy only)
    • Mayor's Permit
    • BIR COR (2303)
    • Owner's ID (with 3 signatures)
    • Expired Business Name Certificate
For Representatives:
  • Authorization Letter with signature from the owner or ...
  • Good copy of Valid Gov't Issued ID
